Following the determination of a finding by the Administrative Hearing Panel, the Complainant or Respondent may submit a written appeal. The Appeal Process (requirements, procedures, appeal grounds) can be found here. An appeal must be submitted in writing using the Request for Appeal form (below) and submitted as directed on the form. The appeal must be submitted within three (3) business days of notification of the findings and include all evidence substantiating the appeal.

IMPORTANT NOTE: Any Request for Appeal not filed within the 3 business day period will be denied.
